Depending on the patient’s condition, the line may be lowered through non-incisional or incisional revision,but if the previous surgical scar remains strong, revision is performed with the Double Line Technique.
The double eyelid line is too highand looks unnatural
Redesign the new linebelow the scar created by the previous surgery
After incising the new line,perform dual upward dissection to release adhesionof the existing double eyelid line
Measure skin thicknessand suture at the appropriate depth
A lower and more naturaldouble eyelid line is created
*Release adhesion of the levator aponeurosis*The preaponeurotic layer is the space betweenthe orbital septum, which encloses the fat, and the levator muscle that opens the eye.
*Release adhesion of the orbital septum*The preseptal layer is the space betweenthe orbicularis muscle just beneath the eyelid skinand the orbital septum surrounding the fat.
Dual dissection (dual adhesion release)Dissect both the preaponeurotic and preseptal layersto release adhesion in two planes and remove the scar of the previous crease line.
Depending on the patient’s condition (multiple creases, hollow upper eyelids, ptosis),fat may be added or removed,and additional procedures such as ptosis correction may be performed.
The previous double eyelid scar is removed,and a new crease line is created below it.
