When you connect the brow bone and cheekbone in profile,
if the eyeball projects beyond that line, it is considered a protruding eye,
and if it sits behind the line, it is considered a sunken eye.

Protruding Eyes Characteristics and Improvement Methods

Compared with sunken eyes, protruding eyes usually create a relatively
larger and more glamorous eye appearance.
The surrounding skin and muscle tissues are also thinner, so there is more freedom when designing
the double eyelid crease.


In protruding eyes, the eyeball supports the drooping eyelid skin and brows,
so compared with sunken eyes, there tends to be less heaviness and less eyelid sagging.
However, as the upper eyelid fat atrophies with age, sagging eyelid skin can build up above the eyeball and appear as multiple eyelid creases.

Because the eyeball pushes the skin outward,
the tension of the epicanthal fold is also stronger.

Upper Epicanthoplasty

causing discomfort. Ptosis correction can help the eyes open more comfortably
while improving them into a clearer, more defined eye shape.

Orbital Fat Grafting

As we age, the upper eyelid fat gradually atrophies,
creating hollow upper eyelids and multiple creases that make the eyes look older and more tired.
Orbital fat grafting can improve this by correcting the hollow upper eyelids and multiple creases

Protruding Eyes VS Sunken Eyes

Sunken Eyes Characteristics and Improvement Methods

With sunken eyes, the tissues around the eyes cover the eyeball,
so compared with protruding eyes, the eyes tend to look relatively smaller.
In addition, the skin and muscle tissues are thicker,
which means there are more limitations when designing the double eyelid crease.

In sunken eyes, the eyeball provides less support against sagging brows and eyelids,
so vertical drooping is more likely to appear.
As aging progresses, sagging upper eyelids can cover and hide the crease,
and upper eyelid heaviness becomes more pronounced.

  • The skin and muscle tissues are thicker
  • There are limits to how high the crease can be designed

A Moderately Heighted Double Eyelid Crease

Because the tissues around the eyes are thicker, designing the crease too high or excising too much tissue
can create a sausage eyelid appearance.
For this reason, the crease should be designed at a moderate height
to create a natural eye appearance.

Canthoplasty

By using canthoplasty procedures such as medial epicanthoplasty, upper epicanthoplasty,
lateral canthoplasty, and lower eyelid surgery,
the surgeon can expand the size of smaller-looking eyes
and refine the eye shape.

Correction of Eyelid Sagging

As aging causes the eyelids to sag, they further cover the sunken eyeball,
making the eyes look even smaller. The cause of each patient's eyelid sagging must be identified,
