Through systematic and precise diagnosis combined with meticulous and accurate surgery, we deliver highly refined surgical results.

Shin Jaehoon Chief Surgeon / Board-certified Plastic Surgeon

A plastic surgeon must see the forest, not just the trees.
If the pre-surgery analysis is wrong, no matter how well the surgery goes,
it can head in the wrong direction.
Many patients come in for double eyelid surgery, but the real issue is not the eyes
but sagging eyebrows and forehead.
Many come for nose augmentation, but the actual cause is a protruding mouth or receding chin.
Minimizing the gap between the patient's and the surgeon's perspective
to achieve the best results — that is my philosophy of plastic surgery.

02

The definition of beautiful eyes varies from person to person. Some think eyes with a double eyelid
are beautiful, while others consider eyes without a double eyelid to be more attractive.

However, as a board-certified plastic surgeon, after consulting and studying countless patients, my conclusion is that
the eye shape that patients seeking eye surgery desire is always the same.

Patients visit the clinic with photos of celebrities and influencers who are popular at the time
of their consultation. The celebrities and influencers whose eyes patients admire are all different
people, yet the eye shape is always the same.

Nowadays, smartphones make it easy to edit photos, so many patients bring in edited versions
of their own photos, yet the desired eye shape remains the same.

WOMAN’S EYES
Women's Eye Shape
Eye shape does not simply refer to the double eyelid as commonly thought, but includes the overall
shape from the inner eye corner to the outer corner, as well as the angle, tilt, and form of both corners
and other detailed elements.

Generally, the upper eyelid line is highest at the center, forming a so-called crescent shape, while the
lower eyelid line follows a nearly straight horizontal line from the inner to the outer corner.

When women apply makeup, they add shading to the outer corner triangle area to make the eyes
appear larger toward the back and downward, and draw the lower lash line to look straight.

This is evidence that we subconsciously know what beautiful eyes look like.

Crescent Eye Surgery — For a Soft Eye Appearance
Crescent Eye Surgery corrects eyes where the lacrimal lake is excessively exposed and the outer corner is upturned creating a fierce look, or eyes where the lacrimal lake is barely visible and the outer corner is tightly closed creating a constricted look, by expanding them fully with Full Canthoplasty and reshaping them into a gentle crescent eye appearance.

If you have complexes about your eye appearance such as small eyes, constricted eyes, or narrow eyes, makeup can cover them to some extent, but there are limitations. If you want a definitive improvement, you can consider eye surgery options like double eyelid surgery, canthoplasty, or full canthoplasty to change the size and shape of your eyes.

MAN’S EYES
Men's Eye Shape
The eyes that men desire are not much different from what women want.

The common features of the ideal eye shape desired by both men and women are horizontally elongated and wide-open eyes,
a refined appearance that does not look fierce, and a clear eye appearance that does not look drowsy.

Men's Double Eyelid: Non-incisional Ptosis Correction and Full Canthoplasty
If you are not actually drowsy but often hear that you look sleepy, you may want to consider ptosis. Ptosis is a condition where the muscle that opens the eyes is weak, preventing the eyes from opening fully, and the drooping eyelid begins to obstruct vision, causing discomfort.

What Do Admired Men's Eyes Have in Common?
Many patients bring photos of celebrities whose eye appearance they admire when they come for consultation. Looking at these photos, there is a common thread. They prefer a thin hidden crease (inner double eyelid) or no crease (monolid) with horizontally wide-open eyes. While women typically want bigger and more beautiful eyes, men's surgical goals are more focused on improving their overall impression.

3. The Key to Beautiful Eyes Is Eye Shape

SISUN PLASTIC SURGERY

Many people commonly think, ’If I get double eyelid surgery, my eyes will look prettier, right?’ or ‘If I make the double eyelids large and glamorous
so the eyes themselves look bigger, they will be prettier, right?’ To this, I would like to say:
”That is not the case.”

80-90% of Koreans have inner eye corners covered by an epicanthal fold, with in-fold or monolid eyelids,
and upturned outer eye corners forming a parallelogram or triangular shape.

18

Korean Eyes

Ideal Eyes

With this type of eye shape, no matter how much double eyelid surgery is performed, the shape itself does not change,
making it difficult to achieve beautiful eyes.

That is why the shape of the inner eye corner, outer eye corner, and the double eyelid crease at both the inner and outer corners
must be changed. Let us take a closer look.

19

Case 1.

Post-failed Medial Epicanthoplasty: Hook Shape, Excessive Lacrimal Lake Exposure,
Indented Scar (Epicanthoplasty Reconstruction)

When patients are diagnosed as needing epicanthoplasty reconstruction surgery, they often express doubt.
’Isn’t that a surgery for people who failed medial epicanthoplasty or those whose eyes are too close together?’
This perception exists because epicanthoplasty reconstruction became publicly known when celebrities appeared on TV
sharing their stories of failed medial epicanthoplasty that had to be reversed.

22

A characteristic of Korean eyes is that the epicanthal fold extends downward, making the
inner eye corner look like a hawk's claw or a hook shape. (Whether from revision surgery
or first-time surgery)

Therefore, by blocking the lower part of the bent inner eye corner and moving it upward
through epicanthoplasty reconstruction, the inner corner can be straightened into a horizontal line,
creating crescent-shaped eyes (the condition for beautiful eyes as described earlier).

Natural-born Epicanthoplasty Reconstruction

Contrary to the perception that it is only a revision procedure, many people in Korea who have never had
medial epicanthoplasty — so-called ‘natural-born eyes’ — also need epicanthoplasty reconstruction.

Case 2.

Inner Eye Corner Covered by Epicanthal Fold

As you can see in the photo, the inner eye corner is covered by the epicanthal fold. In such
cases, not only do the eyes look constricted, but the double eyelid crease line is also hidden. Even with double eyelid surgery,
the desired eye shape and crease line will not be achieved, which is why the epicanthal fold must be removed.

30

② When the epicanthal fold is removed, the lacrimal lake at the inner eye corner becomes moderately exposed, and the 45-degree upward
viewing angle opens up, creating a rounded parabolic shape while revealing the double eyelid crease line,
resulting in an ideal eye shape.

In-fold, in-out, and out-fold lines are important options during double eyelid surgery. Many people think
that simply getting double eyelid surgery alone is enough to achieve their desired crease line.
However, the important factors determining the double eyelid line are 40% the double eyelid surgery itself and 60% the canthoplasty —
that is how crucial the presence of the epicanthal fold is.

To change from an in-fold to an in-out or out-fold line, the epicanthal fold must be removed through canthoplasty.
To change from an out-fold to an in-out or in-fold, reconstruction must be used to reshape the inner eye corner.
As such, ③ successful double eyelid surgery is not simply a matter of the crease line alone,
but the shape of the double eyelid line is determined by the presence and shape of the inner eye corner epicanthal fold.

Case 1.

Constricted, Upturned Outer Eye Corner

When the outer eye corner is tightly closed and upturned, the eyes not only appear smaller but can also give
a fierce impression.

40

In this case, ④ the shape and angle of the outer eye corner can be improved through lateral canthoplasty and lower eyelid lowering surgery.

When performing lateral canthoplasty and lower eyelid lowering surgery, rather than simply making an incision and suturing, the procedure must involve
fixation by pulling and anchoring to the periosteum and capsulopalpebral fascia,
so there is no concern about reattachment or loosening.

Case 2.

Lateral Canthoplasty Complications (Reconstruction Cases)

As the number of lateral and lower eyelid lowering surgery cases has increased, so have the complications from lateral canthoplasty.

47

⑤ Lateral canthoplasty complications can manifest as C-shaped deformities, conjunctiva exposure, unnatural shapes, and more.

To correct these issues, lateral canthoplasty reconstruction must be performed. The scar at the outer eye corner and
exposed mucosa are excised, and the lateral canthal ligament is pulled and fixated to the orbital bone before suturing. At this time,
lower eyelid lowering surgery is performed simultaneously to prevent the eyes from becoming smaller.

Firm Fixation at the Appropriate Strength

Third, fixation must be at the appropriate depth and strength. Even if the double eyelid line height is designed to be equal,

Fourth, normalizing the amount and position of fat. When excessive fat was removed during a previous surgery,

causing multiple creases and other problems. ⑨ Fat within the orbital septum must be repositioned

Correcting Eye-Opening Strength

Finally, ⑩ correcting eye-opening strength is essential. When the eye-opening strength is weak, the double eyelid
crease line appears unclear and the eyes look drowsy. The eyebrow and forehead muscles compensate to open the eyes,
causing secondary problems such as forehead wrinkles or obstructed vision.

Given the long history of double eyelid surgery, many people have undergone the procedure. As a result,

it is necessary to assess your current eye condition — whether it involves a high double eyelid, low crease, multiple eyelid creases, unnatural double eyelids, droopy eyes (ptosis),

⑪ When the eyelid skin is thin with no sagging and minimal fat, the natural adhesion
non-incisional method can be performed. Conversely, when the skin is thick with significant sagging and
excessive fat, the incisional method is performed. When ptosis is severe, ptosis correction is
performed, and when there is eyelid hollowing, orbital fat grafting is used to create ideal double eyelids.
